Anyone who has been vaccinated or tested positive abroad and wants to apply for a European certificate of vaccination or recovery should consult the following explanatory information text and follow the steps described there: 'Applying for a European certificate of vaccination or recovery (DCC: Digital COVID Certificate)'.
The above are 2 distinct online procedures that should not be mistaken for each other. Applications submitted using the wrong procedure cannot be processed.
Vaccinated persons in Luxembourg can submit an online request to receive a new vaccination certificate by post. This vaccination certificate - called "CovidCheck Certificate" in Luxembourg - is an EU Digital COVID Certificate (EU DCC) which :
proves that a person has been vaccinated against COVID-19;
is valid:
in all of the European Union (EU);
in Iceland;
in Liechtenstein;
in Norway;
in Switzerland;
contains a secure QR code that can be read by the applications in the different participating countries (27 EU Member States, Iceland, Liechtenstein, Norway and Switzerland) in order to check that the certificate is genuine.
The request to receive a CovidCheck Vaccination Certificate by post can be made via MyGuichet.lu.
Who is concerned
Anyone who:
has been vaccinated in Luxembourg; and
has a 13-digit Luxembourg national identification number (matricule).
This online application only concerns people who have been vaccinated in Luxembourg and who:
wish to request a new European vaccination certificate in paper form (due to loss, destruction, etc.).

Prerequisites
The person must have been vaccinated in Luxembourg.
Costs
The issue of a paper version of the CovidCheck Certificate is free of charge.
How to proceed
Filing an application
Anyone who has been vaccinated can request a paper version of their CovidCheck Vaccination Certificate online via a form available on MyGuichet.lu.
The online application can be made without a LuxTrust product or an electronic identity card (eID).
When applying online, the vaccinated person must indicate in particular:
their first and last names as they appear on their social security card;
their 13-digit Luxembourg national identification number (matricule);
their e-mail address.
It is important to validate the application online by clicking on "Send".
Applicants receive:
a confirmation by e-mail of the request for the certificate after transmission of the request;
a paper version of the CovidCheck Certificate of vaccination by post a few days after the request.
Unjustified requests are ignored and no mail will be sent.
Good to know
People who do not have computer equipment to apply for their CovidCheck Certificate online, can contact the Health Hotline (Santé) at (+352) 247-65533 from:
Monday to Friday between 8.00 and 18.00; and
Saturday to Sunday between 10.00 and 18.00.
